The CCD Clinic is committed to provide quality healthcare services to students, faculty and staff. It also aims to promote the health of the students by disease prevention, early case detection, referral for interventions and resolution of particular health issues. It provides immediate care to its clients by offering first aid and triage for illnesses and injuries. Additionally, it offers health counseling and education for students, faculty and staff. As well as ensures the school community’s sanitation and healthy living conditions.

OBJECTIVES
- Promote Health and Wellness: Aims to promote holistic health and well-being among the school community through preventive care, disease prevention, health education, wellness programs and healthy lifestyle choices.
- Primary Healthcare Services: Offer primary healthcare services such as physical examinations, routine check-ups, vaccinations, screenings and treatment for common illnesses and injuries.
- Health Education: Conduct lectures, seminars, and campaigns on health-related topics. Develop and distribute educational materials on physical and mental health.
- Promote Mental health: Offer mental health screenings, providing access to counseling services.
- Collaborate with campus partners: Establish partnerships with offices and community organizations including other government agencies to promote student well-being and address health-related issues.
SCOPE OF SERVICES
The clinic provides a wide range of healthcare services, including but not limited to:
- Primary Care/ First Aid treatment
- Health screenings and Assessment
- Medical Consultation and services such as:
- Blood pressure reading
- Fasting Blood Sugar testing
- Immunizations and vaccinations (per doctor’s order)
- Mental Health and Wellness programs
- Health Education and information dissemination
- Emergency care and crisis intervention
- Over the counter and prescribed drug dispensing
- Dental Services such as:
- Dental Check-up
- Tooth extraction
- Oral prophylaxis
- Tooth Filling
- Referral of students with severe medical condition and/or during emergency situations to nearest hospital
Policies and Procedures
- The CCD Clinic will schedule the physical and dental health examination of the students enrolled.
- Medical and dental health record forms will be given to each student to be filled in as part of the health assessment and screening. The forms will be submitted to the school nurse for verification and finally submitted to the doctor on their scheduled medical and dental health examination date.
- Students will be informed prior to the date of the medical and health examination.
- A doctor and a dentist will conduct the physical and dental health examination on the last Monday of the month.
- The results of the examinations are recorded in the Patient’s Health record.
- Any findings and recommendations coming from the doctor or dentist wherein the student needs to undergo the tests (Xray, complete blood count and other laboratory tests), a copy of the result shall be submitted to the school clinic for interpretation and health counseling.
- If the student needs further assessment and management, proper referral shall be provided by the school physician.
- The medical and dental health examination is a requirement for each student for the signing of clearance.
- Minor injuries and minor emergencies are treated in the clinic with required medical facilities.
- Parents/guardians shall be informed on their child’s physical condition if considered necessary.
- All health issues and treatment shall be documented in the Daily Treatment Logbook and the Patient’s health record.
- Daily treatment record shall include the following information:
- Name of student
- Date & Time
- Program & section
- Description of the complaint or the condition requiring treatment
- Treatment given
- Remarks
- The student shall inform the school nurse about any medicine and food allergies and any medication taken at home. Medicine in the clinic is free. It is given only for the first few doses as per standing order.
- The school may allow the student to rest in the clinic or recommend going home to take a rest.
- The nurse will issue a Clinic visit pass or a may go home slip. This will be signed by the Program Head and submitted to the guard before the student leaves the campus.
- The first responder will assess if the student should be sent to the clinic and/or stabilize the student.
- Any person concerned will contact the nurse.
- The nurse should attend to the student. Move the student to the clinic or into a quiet, secured area once assessed.
- Parents should be contacted and informed of the incident.
- The student must be kept in close supervision and monitored until recovered or until parents/guardian arrives.
- If the school is unable to contact parent and the student requires further treatment, he/she shall be taken to the nearest hospital and is accompanied by assigned school personnel. Efforts to contact the parents/guardian of the child should continue.
- The first responder should assign someone to call for the nurse.
- School Nurse shall attend immediately and make necessary interventions to the student.
- Emergency Medical Services or 911 shall be called immediately.
- The parents/guardian of the student shall be informed of the student’s condition and to meet their child at the hospital.
- An assigned school personnel shall escort the student in the ambulance to the hospital.
- The accompanying personnel must stay at the hospital until parents/guardian has arrived and all procedures are settled.
- An incident form must be completed by all witnesses and the school nurse which will be signed by the Program Head within 24 hours.
- The school nurse will follow up with parents through phone call until the student is discharged from the hospital. All information and updates about the student shall be documented and attached accordingly to the student’s school medical file.
- The Program Head or Adviser must provide a list of students before securing a medical certificate to the school nurse.
- The school nurse will coordinate the schedule with the physician. This will be a first come, first serve basis.
- The physician will then issue the medical certificate.
- In case the student has some health-related issues, the physician or the school nurse shall notify the Program Head, Adviser or any personnel in charge of the activity and the parents/guardian.
- If the student insists on joining the activity and there was no issuance of medical certificate due to his/her medical condition. The student and the parents/guardian shall submit a notarized document/waiver indicating that CCD is not liable for any medical complications or injuries that may occur during the event for legal purposes.
Student Medical and Dental Health Examination
CCD Clinic encourages all new students to undergo the annual medical and dental health examination to update them with their current health conditions.
First Aid, Medical and Minor Emergency
The school clinic shall be equipped with appropriate medical equipment, supplies and medicines which are required to provide first aid and other emergency services.
Accident and Medical Emergencies
Accidents that do not require immediate transfer to hospital>
In an event that a student is involved in an accident or incident that requires more than the basic first aid interventions the following steps should be followed:

Incidents and accidents that require immediate transfer to hospital are classified as any injury that is life threatening or anything that there is a possibility of incapacity.
Issuance of Medical Certificate for Off-campus Activities
For students attending off-campus activities wherein they need to submit a medical certificate
Medical Health Record
Students’ medical and dental health records are only accessible to clinic personnel and authorized school staff. Each and every record is kept private and confidential.
Medical and dental health file include the following information but not limited to:
- Health history including chronic conditions
- Medical and dental health forms
- Health examination reports
- Medication administrations, needed consents
Clinic activities, medication administration and patient care is recorded in the Daily Treatment Record and in the Permanent Patient’s Health Record.
The health record shall be maintained in school for a minimum of one year after students leave the school.
